New Patient Registrations
Looking for a doctor?Please contact the practice and arrange to complete a new patient registration form and health questionnaire.  You will need proof of identity and proof of address with you when you register.

Late Night Clinics
We are pleased to announce that we now hold late night appointments up until 7:30pm every Monday night. Please note that reception is closed to general queries and prescription collections.

Please try to keep your appointment and arrive at the surgery in good time. If you are unable to make your appointment, please tell us as soon as possible so we can offer your appointment to another patient.

Out of Hours
If you need to see a doctor outside normal surgery hours, please telephone the normal practice number – 01274 224 888 – and you will be directed to our emergency on-call doctor service.  Alternatively, you can call 111 for any health queries.

Test results
Please telephone after 2pm for test results.

Patient Registration

Patient Registration with the Practice

Any new patient(s) wishing to register with the Practice will be required to visit the surgery to complete a new patient registration form and a new patient health questionnaire form.  Both forms need to be signed by the individual patient wishing to register (children's registration forms can be signed on their behalf by a parent or legal guardian).   The patient can request a New Patient Medical Check or with a Nurse.  Before you fill the forms in we need to see proof of identity (driver's license, passport etc) and proof of address (utility bill etc). 

Once registered it can take a number of weeks before we receive your medical records therefore it is important that the medical questionnaire is fully completed.   

Due to the length of time it can take to receive your medical records, it is important that you register with the practice as soon as possible and not leave it until you are ill or need our services.

If you require Repeat Medication immediately, please provide us with one of your prescriptions.  As your medical records are not immediately transferred to us when you register, we require some written evidence of the medication you take before we can prescribe.

Our medical practices work within a specific catchment area.  If you have any doubts whether you are in our catchment area please contact the practice.
